Proctor Participation Agreement Form

Upon completion of all lines listed below, please submit this form in order for the FDTC Testing Center to fulfill the requested proctoring services.

Requirements for Testing:

• Submission of the FDTC Proctor Participation Agreement Form
• Instructions sent from home institution to FDTC for test access - Information can be sent via email to testingcenter@fdtc.edu by the faculty
• Receipt from FDTC Business Office  - 
• Fee $25 (for students unaffiliated with the South Carolina Technical College system)
• Fee $20 (for students affiliated with the South Carolina Technical College system) 

Fee is payable to the Business Office - Main Campus, 5000 Building, 2nd Floor
